MENTOR Oregon is looking for a full-time bilingual Personal Agent in Eugene, OR. Mentor Oregon Brokerage provides service coordination for adults who experience intellectual and developmental disabilities. Mentor Oregon Mid-Valley Brokerage believes in self-determination.  We are a team of employees who work well together to ensure services are delivered in the ways customers want them.


A Personal Agent coordinates services for up to 39 individuals living in their own homes. Personal Agents use a person-centered approach to assess the individual strengths, needs, and preferences of each customer, then develop individual support plans (ISPs) with the person to meet their needs and work toward their goals. Personal Agents coordinate services for customers by developing the customer’s natural support network and by connecting them to community resources and paid support providers.  Personal Agents are responsible for regular monitoring of ISPs, addressing customers' changing needs (both immediate and long term), as well as ensuring customer choice and satisfaction with the services they receive. Personal Agents manage Medicaid funds and monitor ISP services and expenditures.

  • Knowledge of the public service system for developmental disabilities services in Oregon and at least:
    -  A bachelor’s degree in behavioral science, social science, or a closely related field; or
    - A bachelor’s degree in any field and one year of human services related experience; or
    - An associate’s degree in a behavioral science, social science, or a closely related field and two years of human services related experience; or
    - Five years of human services related experience.
  • Strong communication skills, including both written and oral forms
  • Ability to read and write fluently in both English and Spanish
  • Skills in meeting facilitation and conflict resolution
  • Ability to work independently and efficiently with attention to detail, demonstrate strong time; management skills, organizational skills, strong math and budgeting skills, meet established deadlines, as well as have the ability to adapt to change;
  • Strength in comprehending, interpreting, and implementing a wide variety of established policies, procedures, and the Oregon Administration Rules;
  • Proficiency with Microsoft Office computer programs, and comfortable with use of a database;
  • Commitment to choice for individuals receiving services and focus on self-determination;
  • Prior experience in service coordination preferred

Conditions of Employment:
    • Must pass a criminal background check
    • Must have access to a working vehicle, and the ability to safely and efficiently meet customers in their homes and other community settings
