MENTOR Oregon is looking for a Lead Personal Agent/ Case Manager in Salem, OR.  The Lead Personal Agent trains, supervises, and monitors Personal Agents to support them to be successful in their jobs.  The Lead Personal Agent works cooperatively as part of the brokerage management team.  The Lead Personal Agent contributes to the analysis of staff trends, policy development, and to the general improvement and overall functioning of the brokerage.  



Description

Responsibilities include:    
  • Directly supervises 6 full-time Personal Agent/Case Managers (PAs)
  • Reviews and provides feedback on a sample of supervisees’ billable progress notes each month
  • Responsible for hiring, onboarding and ongoing training for PA staff
  • Reviews all ISPs, Service Agreements and Adult Needs Assessments completed by PA supervisees for accuracy and completeness
  • Completes quality assurance activities to monitor PA performance and to identify training needs;
  • Reviews and helps to complete Crisis Entry Summary form and other Crisis documents for all supervisees; provides process support and oversight to PAs
  • Reviews and submits Exception Requests to the ODDS Exceptions Committee, coordinates on additional information requests
  • Helps as a member of the Brokerage Management Team to implement systemic changes to policy and practice
  • Participates in regular internal and external meetings as part of the Brokerage Leadership Team



Qualifications:

  • An undergraduate degree in a human services field
  • At least 3 years of case management experience
  • Brokerage knowledge and knowledge of the public service system for developmental disability services in Oregon required.
Preferred:
  • Supervisory experience is highly preferred
  • Excellent written and oral communication skills
  • Strong interpersonal skills and experience in mediation or conflict resolution
  • Proficiency with Microsoft Office computer programs
  • Proficiency in basic math skills, and the ability to apply them to budgetary and other planning functions
