Description As a Program Services Coordinator you will: 

• Assess referred clients and coordinates the development plan in conjunction with a treatment team and funding source in accordance with Corporate, contractual and regulatory standards
• Conducts home visits bases on individual need and completes progress notes
• Oversees the implementation of the Individual Service Plan (ISP); ensures on-going communication with planning team and Program Director
• Works with Program Director to oversee and provide consumer care and supervision, as needed
• Builds and maintains relationships with families and Regional Centers
• May be responsible for managing and responding to calls after normal business hours of assigned caseload for crisis intervention support


• Bachelor’s degree in social services or related field and two years’ experience supporting individuals with intellectual and developmental disabilities
• Travel as needed to assigned caseload
• Strong follow through to ensure quality services, regulations and clients’ rights are upheld
• Valid driver’s license in good standing required, access to a registered vehicle with proof of insurance
• Successful clearance of local and/or state background checks
• Compassionate approach and reliable, responsible attitude
